A structured framework for improving collaboration between practice nurses, GPs, and other services in the assessment and management of patients with cognitive impairment presenting in General Practice

Project Description

This project aims to train GPs and nurses working in general practice in the assessment of people with possible cognitive impairment for cognitive function, activities of daily living, instrumental activities of daily living, depression and anxiety, and also to assess carers for carer burden.

These assessments will contribute to identification of dementia by either nurse practitioner, the memory clinic or the GP. In addition nurses will be trained in the pathways of referral to local community support agencies, and to Alzheimer's Australia.

Nurses will then work with GPs under the care plan nursing item numbers to regularly review patients with a diagnosis of dementia and implement a plan of care involving assessment and referral to appropriate agencies, using algorithms derived from the assessment instruments.
